gender on questionnaires

today i participated in a linguistics study about gender, and the attached questionnaire had exceptionally nuanced and responsibly designed questions about participants' gender, in three parts. this is how i remember it (although i forgot the exact wordings):
1. What sex was assigned to you at birth?
– male
– female
– other/intersex
2. Do you identify as transgender or transsexual, have you transitioned in the past or do you plan to?
– yes
– no
– I don't know
3. Do you identify as nonbinary?
– yes
– no
– I don't know
of course i don't love the first question, which doesn't mean i don't think it should be there. i think it's very well designed, one of the best i've ever seen. of course i considered taking a screenshot, but thought better of it because what would i do with it anyway? well, now i wish i could have another look at the formulation of the questions.
